For the 2017 calendar year, CMS has announced that they will operationalize the new coding rules, including adoption of the new code descriptors for CPT codes 77065, 7707 through the use of the three current HCPCS codes G0206, G0204 and G0202.

For free-standing and hospital-based clinical laboratories only, Emblem Health will recognize Healthcare Common Procedure Coding System (HCPCS) codes G0477 through G0483 for drug testing services, consistent with CMS coverage guidelines.
Surgical pathology on four or fewer prostate needle biopsy specimens should be reported with CPT code 88305 with the unit of service corresponding to the number of separately identified biopsy specimens.
